Initial assessments

We work closely with individual service users, their families, service providers, and other healthcare professionals in obtaining background information to help us formulate person-centered care plans on all new care packages and on care plan reviews. Initial service user assessments will be carried out in the comfort of their own homes to allow the assessor to carry out environmental risk assessments and ensure the safety of our clients and staff. Service users’ personal circumstances and support requirements are reviewed and will help the assessor to formulate care plans which are bespoke to each service user.

Service users’ personal choices and preferences will be well documented in individual care plans and these will be reviewed regularly to ensure all agreed and changing care needs are met.

Caring through empowerment

We aim to empower our service users to continue to meet their day-to-day care needs independently as much as is possible through supervising and exercising patience during their scheduled care times. We will offer support to individuals requiring more support needs from our staff to help meet their agreed care goals. Service user care/ support plans will be audited monthly to ensure the agreed goals are fully met.  We aim to deliver high-quality, person-centred care needs to the satisfaction of our service users.
